New Services Application 

1.  Open the form by right clicking on the following link and clicking “SAVE LINK AS / SAVE TARGET AS”.

2. After downloading the form, open it, fill out all required fields and save your changes!

3. Attach and send by email to aeeservices@adaktu.net

4.  Click on “Pay Now” to pay deposit, install, or work order fees.  You will be redirected to a secure Paypal site which accepts all major credit cards.  You can also pay over the phone: dial (1-907-222-0844) or dial (611) while on Adak Island.
